This policy explains what we collect, why, and what we will never do with it. It covers upperboundrx.com and app.upperboundrx.com.

What we collect

• Contact and account details: your email, name, phone, and shipping address.
• Quiz and eligibility answers: your goals, state, age range, and the qualifiers you provide before purchase.
• Health information: your medical intake, which is collected to enable your physician to care for you. Health information handled by your treating physician and pharmacy is protected health information governed by HIPAA and their own notices of privacy practices.
• Payment details: processed by Stripe. We never see or store your card number.
• Usage events: first-party, cookie-free counters of how the site and funnel are used (for example, that a quiz was started). These contain no advertising identifiers and are not shared with ad networks.

How we use it

To route your intake to a licensed physician in your state, to fulfill what they prescribe through a licensed pharmacy, to run billing and your member dashboard, to communicate with you about your care and account, and to improve the service. That is the whole list.

What we never do

• We do not sell your personal information, and we do not share it with data brokers.
• We do not share your health information with advertising platforms.
• We do not send your data to anyone who is not directly involved in your care, fulfillment, payments, or legal compliance.

Who we share with

Your physician and their care team, the fulfilling pharmacy, our payment processor (Stripe), our infrastructure providers (hosting and email delivery), and authorities where the law requires it. Each receives only what their role requires.

Retention and your rights

We keep account and care records as long as the law and medical record-keeping standards require. You may request access to, correction of, or deletion of your personal information (subject to records we are legally required to keep) at support@upperboundrx.com.

Security

Data is encrypted in transit, access is limited to those who need it, and health details live in systems designed for medical data. No system is perfect; if a breach affects you, we will tell you promptly and plainly.
