This document explains what it means to receive care through Upperbound's telehealth platform. You accept it when you submit your medical intake.
Your care begins with a written intake that a licensed physician in your state reviews personally, usually within 24 hours. The physician may message you with questions, request more information, recommend in-person evaluation, approve a pathway, or decline. Care continues through scheduled check-ins at weeks 6 and 12 and through messaging in your dashboard.
Telehealth gives you access to physician care without visits, at the cost of a physical examination. Your physician makes decisions from what you report, so accuracy and completeness in your intake are part of your care. If the physician believes your situation requires in-person evaluation, lab work, or is not appropriate for telehealth, they will tell you instead of prescribing.
A physician-patient relationship is formed between you and the treating physician when they accept your case, not with Upperbound itself. The physician exercises independent medical judgment and is responsible for your clinical care. Upperbound provides the platform, billing, and logistics.
As with any medical care, prescribed treatment can have side effects and may not produce the outcome you hope for. Telehealth-specific risks include the limits of care without physical examination and the possibility of technical failures delaying communication. Information transmitted electronically is protected as described in our Privacy Policy.
Telehealth is not for emergencies. If you experience a medical emergency at any point, call 911 or go to the nearest emergency room. Tell any emergency provider about treatments you are receiving through Upperbound.
You may stop treatment, cancel your pathway, or request your records at any time. Declining to consent to telehealth simply means we cannot provide care through this platform; it will never affect your right to seek care elsewhere.
